Social Prescribing is a non-clinical service aimed at supporting patients alongside the medical expertise to address the root causes of their health conditions.

The Active Wellbeing Society has begun broadening its offer of online support groups for patients at Firs Surgery.

The service can also offer some face to face interventions for those who cannot access, or would not benefit from online support, particularly where practices have highlighted a clear health priority. The service has adapted the support to ensure all activities follow COVID-related guidance to minimise viral-transmission and protect the safety of patients and staff.

The Social Prescribing service:

  • Help and support non clinical aspects of health and wellbeing.
  • Is personalised with a focus on what patients want to achieve.
  • Linking to other services such as mental health support, housing, benefits, employment, courses etc. Help to get patients involved with local community groups and local services.
  • Provide patients with the time to discuss things that matter to them.
